Yes. Your Long-Term Care Insurance Company Case Manager is still managing your long-term care case, we are simply assisting them in providing you with optimal care and services.
Does joining this program cost anything?
There are no charges for this service. Once you become a member of Faith Health Care, you are a part of our family.

Is this Program meant to replace an insurance company case manager?
Not at all. This program is meant to support the Long Term Care Insurance Case Manager in their role in order to provide our mutual Patient with a full 360 range of care, where their needs are being regularly assessed and addressed by both the LTC case Manager and the Faith Health Care Optimal Care Coordinator.
