This Privacy Notice (“Notice”) explains how FaithConnect, a mobile and web platform operated by Faith Healthcare (“FaithConnect,” “we,” “us,” or “our”), collects, uses, and shares information about individuals who provide—or start or complete an application to provide—caregiving services as independent contractors through the FaithConnect mobile app and our website at www.faithhealthcare.com (together, the “Services”).
Public availability. FaithConnect is publicly available to independent caregivers. No invitation, managed device program, or enterprise enrollment is required to register and use the app’s public features (including registration and in-app messaging). This Notice applies to caregivers and applicants; it does not apply to patients/clients receiving care.
Information We Collect
Information you provide
- Registration and profile. Name, phone number, email address, password or other login credentials you create to register and access the app.
- Communications. Content of messages you send via in-app messaging or email, and your responses to prompts or forms within the app.
- Verification. Information used to verify identity or eligibility where required (for example, one-time passcodes sent by SMS).
Information collected automatically
- Device and usage. IP address, device identifiers, operating system, app version, log files, crash/diagnostic data, pages/screens viewed, and interactions with the Services.
- Cookies/SDKs. Technologies that support sign-in, security, and analytics.
We do not intentionally collect patient/client medical information from caregivers through the app.
How We Use Information
- Create and manage accounts, enable registration and in-app messaging, and provide the Services you request;
- Authenticate and secure accounts (including SMS/one-time passcode verification), prevent fraud and abuse, and protect the platform and its users;
- Operate, maintain, and improve the app and website, including diagnostics and analytics;
- Communicate about the Services, respond to inquiries, and provide support;
- Comply with law, enforce our terms, and protect rights, safety, and security.
We do not sell or rent personal information.
When We Share Information
- Service providers that host the app/website, deliver messaging/SMS, provide analytics, security, and customer support;
- Compliance and safety purposes (to comply with law, respond to lawful requests, enforce terms, or protect users and the platform);
- Aggregated or de-identified information that does not reasonably identify you.
- SMS consent will not be shared, sold, or disclosed to any third party or any other individual.
Your Choices
- Access and updates. You may access and update certain account information in the app or by contacting us.
- Email preferences. You may unsubscribe from non-essential emails by following the instructions in those messages or contacting us at the email below.
Children’s Privacy
The Services are intended for adults who are eligible to provide caregiving services under applicable law. We do not knowingly collect personal information from children. If you believe a child has provided information through the Services, contact us and we will delete it.
Security
We use administrative, technical, and physical safeguards appropriate to the nature of the information we process, including encryption in transit, access controls, monitoring, and regular reviews of our practices. No method of transmission or storage is completely secure.
International Use
If you access the Services from outside the United States, your information may be processed in the United States, where privacy laws may differ from those in your jurisdiction.
Changes to This Notice
We may update this Notice from time to time. We will post the updated version with a revised date and, if changes are material, provide additional notice as appropriate. Your continued use of the Services after an update signifies acceptance of the updated Notice.
